§1 General information
The Klik Laser studio is a brand operating within MODESTIC Krystyna Zaitseva, registered at MODESTIC Krystyna Zaitseva, ul. Głębocka 5/U10, 03-287 Warszawa, NIP: 8133873866, REGON: 521112715. Contact: +48 733 640 892, info@modestic.pl.
The studio is not a medical entity and does not provide health services. The treatments offered are aesthetic-cosmetology treatments performed using a picosecond laser.
§2 Scope of services
At the studio we provide:
- safe laser tattoo removal
- safe permanent makeup removal (brows, eyeliner, lips)
- initial consultations before treatment begins
The current price list and service descriptions are available at kliklaser.pl and at the studio.
§3 Booking a visit
A booking can be made via the kliklaser.pl website, by phone on +48 733 640 892, or in person at the studio. A booking requires a first name and phone number, plus a one-time verification of the number by SMS code.
To the phone number provided we send: a booking confirmation, an appointment reminder (24 hours before), and notifications of rescheduling or cancellation.
§4 Cancellation and rescheduling
A visit may be cancelled or rescheduled free of charge no later than 24 hours before the planned time — via the kliklaser.pl website or by phone. In the case of cancellation less than 24 hours in advance, or a no-show, the studio reserves the right to charge the booking fee.
§5 Course of the visit
Before the first visit the client completes a health-interview form and a treatment-consent form. The person performing the treatment may decline to provide the service in the case of:
- refusal to sign the required documents
- failure to meet safety indications (e.g. fresh tan, pregnancy)
- identification of medical contraindications
- attending in a state that prevents the treatment (alcohol, intoxicating substances)
§6 Client rights and obligations
The client has the right to:
- respect for personal dignity
- reliable information about the course, effects and possible risks of the treatment
- give or refuse consent to individual stages of the treatment
- service by appropriately qualified personnel
The client undertakes to:
- follow the pre- and post-treatment recommendations
- report any changes in health between visits
- attend visits punctually
- behave courteously at the studio
§7 Payment
Payment for the service is made at the studio on the day of treatment. We accept cash, payment cards and BLIK. At the client's request we issue a VAT invoice — please provide invoice details before payment.
§8 Documentation
The studio keeps records of the client's visits in accordance with the Privacy policy. Documentation is made available at the client's request without undue delay, no later than 7 calendar days from the request. The first copy is free of charge.
§9 Complaints
Complaints regarding a service performed should be submitted in writing to info@modestic.pl or at the studio. A complaint is considered within 14 days of receipt.
§10 Protection of personal data
Clients' personal data is processed in accordance with the Privacy policy and applicable law.
§11 Final provisions
In matters not governed by these terms, the relevant provisions of Polish law apply. The terms are made available at the studio and on the kliklaser.pl website. Clients will be informed of any material changes via the website and by SMS.
